There are two beneficiary types –Trust Nomination and Revocable Nomination. A Trust Nomination means you’ll give away all rights to your policy ownership. This means you can’t make any policy changes without getting your beneficiary’s approval. A Revocable Nomination means you can make policy changes without your beneficiary’s approval.

To make a nomination, simply download and fill up our Revocable Nomination form or Trust Nomination form. The completed and signed original form must be submitted by hand or post to FWD Singapore Pte. Ltd. 6 Temasek Boulevard, #18-01 Suntec Tower 4, Singapore 038986 for our registration.
